European forms
Below you will find the list of the european documents of the Exxx type that are mostly used.
The documents can be obtained from the last country in which you performed paid work. (see list of countries)
E 101
A posted worker is a person who works on for a limited period in a territory of an European state member which is not the country where you usually works.
The E 101 is delivered when both countries agree to send a person to work as a posted worker in another EEE country or in Switzerland.
The E 101 has to be delivered by the country where you pay for social contributions.
Once you get the E 101 document, you can ask to your social security for the E106 document. This document will open your rights for health care in the country you are going to be posted.
E 106
Worker on temporary assignment:
You work in Belgium but you still pay for social security in another European country: Embassies / Missions / Delegations / International Companies /...
Border worker:
You work in a bordering country and live in Belgium.
With the E 106 it is possible to transfer to Belgium rights of a worker who remains subject to the legislation of another European country.
Based on the E 106 document, you are covered (emergency and non-emergency care) as per Belgian conditions and at Belgium rate. You should submit it to Partenamut at the same time as your registration. As soon as your registration is agreed, you will receive a SIS card as well as identification stickers (this card and stickers are your social security identifications).
In order to register, you only have to fill in the online membership form, sign it and send it back to us together with your E 106 document.
E 109
This document is for dependant people only (spouse/husband, children, …) who lives in another country than the holder. The holder works in a European country.
For example: The husband works in France and the wife lives in Belgium with the two children.
With E 109 document, you are covered (emergency and non-emergency care) as per Belgian conditions and at Belgian rate. You should submit it to Partenamut at the same time as your registration. As soon as your registration is agreed, you will receive a SIS card as well as identification stickers (this card and stickers are your social security identifications).
In order to register, you only have to fill in the online membership form, sign it and send it back to us with your E 109 document.
E 111 / EHIC (European Health Insurance Card)
With the EHIC Card you can get access to health care in a country of the EEA (or in Switzerland) during a short-temp stay.
Short-term stay:
- holidays
- studies
- trainees / work placements
- special cases
Note: The E 111 or European Health Insurance Card is not valid for a scheduled treatment in Belgium (see E112 document).
In order to register you only have to fill in the online membership form, sign it and send it back to Partenamut together with your E 111 document or European Health Insurance Card.
Are you going on holiday soon, overseas under the sun or for snow sports? Is your daughter going to Florence for the next 5 months? Thanks to your registration to the Complementary Insurance, and if your payments are in order, your Mutuality’s Emergency Abroad Service (Soins Urgents à l’étranger - SUE) can bring you assistance.
Medical Emergency Assistance
“Mediphone Assist” allows you, wherever you are, to benefit from a medical assistance and repatriation centre. Mediphone Assist guarantees a top quality assistance (free repatriation in case of emergency), but also an information service.
Mediphone Assist is our new Overseas Assistance permanence. Operated by Mondial (Worldwide) Assistance, you can contact it 24/7 in case of unexpected Medical emergency while you are staying abroad, in most European countries and some Mediterranean countries.
What does Mediphone Assist covers?
- Payment guarantees in case of Emergency Hospitalisation, dialysis, oxygen therapy, or Emergency ambulatory care, above 500 Euros.

Furthermore, you can contact Mediphone Assist from any country in the world for any query related to Hospitals, doctors, dentists, nurses and chemists abroad.

Complementary Financial Interventions
In case of abroad medical cares, your mutuality pays into your account the amount set by the Compulsory health Insurance from the country of your stay.
- Your registration to the complementary insurance also allows you also to get a extra intervention in case of expenses below:
- Medical expenses in case of Hospitalisation: only for the first 15 days, unless the return journey is impossible because of essential medical care on location
- Expenses linked to ambulatory care: if expenses are less than 500 Euros, an extra 25 € charge is payable. How to get reimbursed? As soon as you are back with proofs of payments

New! Extension of the coverage period
The service cover now lasts 6 months from the departure date. It is extended to 12 months for students taking part to an exchange programme or a training course in a European Union state member or In Norway, island, Liechtenstein or Switzerland.

If you wish to go to a country that is not in the list, we advise you to subscribe to an Assistance Insurance from an Insurance Company to benefit from a required Cover for Emergency Medical care and Repatriation.
Assistance and Reimbursement Rights
If you don’t follow the Assistance Permanence directives and processes, all intervention will be denied. The assistance centre has to be informed within the 48 hours following the Hospitalisation and / or the accident.
E 112
It allows a foreign policy holder to receive a scheduled treatment in Belgium.
The interested party has a right to all risks (emergency and non-emergency care) as per Belgian conditions and Belgian rate.
In order to register, you only have to fill in the online membership form, sign it and send it back to us together with your E 112 document or European Health Insurance Card.
E 121
This allows the holder to benefit from a retirement pension provided by legislation of another country in the EEA (and Switzerland). It also allows his/her dependents to benefit from health care reimbursements in Belgium when they transfer their residence.
With E 121 document, you have rights to all risks (emergency and non-emergency care) as per Belgian conditions and at Belgian rate. As soon as your registration is agreed, you will receive a SIS card as well as identification stickers (this card and stickers are your social security identifications).
In order to register, you only have to fill in the online membership form, sign it and send it back to us together with your E 121 document.
E104
With E 104 document, you have the possibility to transfer specific rights in order to avoid qualifying periods in Belgium. To get compensation in case of a long-term illness (more than one month), there is always a trial period of 6 months. Under specific conditions this trial period can be cancelled with the E104.
